The SBH Ambulatory Care Center is SBH Health System’s largest outpatient facility, offering a full range of adult, adolescent and pediatric primary care and specialty care services. Our goal is to promote health and wellness for individuals and families living in the Bronx community. We want our patients to thrive under our care.

The Center’s designation as an NCQA Patient-Centered Medical Home means that our patients benefit from the newest and best model of care available. It means that patients are treated as individuals. They not only have their own personal doctor, they have a whole healthcare team dedicated to their care. In other words, we provide healthcare that focuses on the patient as a Whole Person.

At our Ambulatory Care Center patients will find highly trained primary care doctors and specialists offering a full scope of services including surgical, cardiac, cancer, and high-risk prenatal care. The Center is also home to Pathways, our designated AIDS center and the Men’s Health Center dedicated exclusively to men’s health issues. Behavioral Health, nutritional, social work support and care management services are also provided.

Along with a dedicated doctor and healthcare team, the Center offers patients the convenience of on-site laboratory and radiology services.

The Ambulatory Care Center is also home to our WIC program that provides low-income women, infants and children under five years of age with nutritional support, nutrition education and other services.

For patients with questions or concerns about their insurance status, we have highly trained financial counselors available to assist with information about health insurance options.

The parking garage at Queens Park Hospital is open 24 hours and 7 days a week for all patients and visitors.
